Long-term care (LTC) is a variety of services which help meet both the medical and non-medical needs of people with a chronic illness or disability who cannot care for themselves for long periods of time.
It is common for long-term care to provide custodial and non-skilled care, such as assisting with normal daily tasks like dressing, feeding, using the bathroom. Increasingly, long-term care involves providing a level of medical care that requires the expertise of skilled practitioners to address the multiple chronic conditions associated with older populations. Long-term care can be provided at home, in the community, in assisted living facilities or in nursing homes. Long-term care may be needed by people of any age, although it is a more common need for senior citizens.

Respiratory Therapy Services Offered In-House
All In-House Respiratory Services provided or consulted by a licensed and Nationally Certified Respiratory Therapist with Pulmonology Services available.
Sub-Acute care is comprehensive inpatient care designed for someone who has an acute illness, injury, or exacerbation of a disease process. It is goal oriented treatment rendered immediately after, or instead of, acute hospitalization to treat one or more specific active complex medical conditions or to administer one or more technically complex treatments, in the context of a person's underlying long-term conditions and overall situation.
Sub-acute care is generally more intense than traditional nursing facility care and less then acute care. It requires frequent (daily to weekly) recurrent patient assessment and review of the clinical course and treatment plan for a limited (several days to months) time period, until the condition is stabilized or a predetermined treatment course is completed.
Our team of licensed therapists meet with the resident, physicians, clinical care givers, and families to develop an individualized short term rehabilitation treatment plan for each resident. We encourage families to participate in the therapy sessions to experience how their loved one is doing.
In most cases aggressive daily treatment is the key to a speedy recovery and getting your loved ones home as quickly as possible. That's what Four Seasons main objective is once you become a patient.
Autumn Lane Memory Care Unit is a 40-bed secured memory care unit at Four Seasons. Our staff is committed to servicing the needs and lifestyles of the residents in this community, as well as meeting and exceeding the expectations of our residents and their families.
Specialized Memory Care Services
Our recreational activity department has developed a program to stimulate the cognition of our residents who may be suffering from mental illness such as Alzheimer's disease and dementia.
